Important Information Regarding Your 2022 1095B Tax Form

Due to a change in the tax law, the Fund will not be automatically mailing you a 2022 1095b form. Effective with the 2019 tax year, the IRS penalty was reduced to zero for individuals with no health coverage, therefore the form is no longer required in order to file your federal income tax return. However, a request for a paper form can be made by contacting the Fund Office at (844)347-IBEW (4239) or by mail at IBEW Local 347 Electrical Workers Health and Welfare Fund, P.O. Box 26068, Salt Lake City, UT 84126-0068 or by email at mkemp@compusysut.com.

IBEW Local 347 Electrical Workers Health and Welfare Fund is pleased to announce a new Employee Assistance Program (EAP) called LifeMatters! We have contracted with Empathia, Inc. to provide this free, confidential service to you and your immediate family members.
